What is MACRA, the QPP and MIPS/Advanced APMs? 
MACRA is the Medicare Access and CHIP Reauthorization Act of 2015 aims to change how clinicians are paid: it ended the Sustainable Growth Rate formula and implemented changes to pay clinicians based on quality, instead of volume, of care. MACRA created the Quality Payment Program (QPP), which CMS says will improve Medicare “by helping you focus on care quality and the one thing that matters most — making patients healthier.”

The QPP has two tracks you can choose to participate in: the Merit-based Incentive Payment System (MIPS) or the Advanced Alternative Payment Model. 

Most of our Amazing Charts providers fall into the MIPS category. If you participate in an Advanced APM, through Medicare Part B you may earn an incentive payment. If you participate in MIPS, you earn a positive or negative performance-based payment adjustment.

MIPS scores providers based on four categories and uses those scores to determine payments. Those four categories are: 

  • Quality Measures (60% of total MIPS score)
  • Advancing Care Information (25% of total MIPS score)
  • Improvement Activities (15% of total MIPS score)
  • Cost/Resource Use (This category will not used in calculations for adjustments for 2017, but will be analyzed)

Clinician performance will be measured annually and will be made available to the public via CMS. MIPS payment adjustments will apply to Medicare Part B payments starting in 2019, based on 2017 performance reporting.
